IAHI 360° Leadership
Survey Checklist
 

  • Appoint a point person in the organization, preferably a person in HR, who will act as project manager for the administration of the IAHI 360 Leadership Survey.
  • Review supporting documentation for IAHI 360 Survey: IAHI 360 Survey Questions & Report sample.pdf. Determine most appropriate application of 360 in your business.
  • Educate leadership team on use and process of the 360 Survey.
  • Identify number of people who will be participating in the IAHI 360 Leadership Survey. Obtain approval for total survey cost. 2006-2007 pricing: $95.00 per person receiving a 360 Feedback Report.
  • Determine delivery date for 360 Leadership Feedback Report(s).

3 weeks prior to 360 Leadership Feedback Report delivery date:

  • Contact Janet Langley at Lockwood Leadership International at 864-962-6789 or via e-mail at janet@lockwoodleaders.com.
    • Specify that you will be administering the IAHI 360 Survey process in your business.
    • Verify current pricing and adjust budget or desired activity if necessary
    • Indicate the maximum number of Rater surveys to set up for each participant
    • Coordinate survey launch date and deadlines for completion of the IAHI 360 Survey and subsequent delivery of reports.
    • Communicate invoice / billing contact and process.
    • Provide all participant names and e-mail addresses to Lockwood.
  • Communicate to participant(s):
    • The intention of the IAHI 360 Survey (e.g. to be used as a personal development tool, to be used as part of a coaching engagement, etc.)
    • The process for completing the IAHI 360 Survey: Participants must select Raters, the desired number and minimum number of Raters, deadline dates.
    • Expect instruction e-mails from Lockwoodleaders@mindspring.com.

At least 2 weeks prior to 360 Leadership Feedback Report delivery date:

  • Communicate launch of survey.
  • Monitor status of surveys being completed on a weekly basis and communicate Rater activity to participant(s).

1 week prior to Feedback Report delivery date:

  • Coordinate with Lockwood Leadership the cut-off date/time for survey activity (based on Rater activity).
  • Establish printing and shipping date for 360 Leadership Feedback Report.
  • Provide Shipping address and Fed Ex account billing information to Lockwood.

Lockwood Leadership Deliverables

Upon work order request by client, Lockwood Leadership International will provide the following relative to Lockwood Leadership Assessment ® survey reports:

  • Client sends Lockwood Leadership International the survey participant names and e-mail addresses. Client should notify survey participants to be expecting e-mails from Lockwoodleaders@mindspring.com.
  • Coordinate deadline with client for completion of the Lockwood Leadership Assessment reports.
  • Confirm with client the number of rater surveys to set up for each participant.
  • Set up group roster for electronic access. Includes participant user i.d. and password assignment.
  • E-mail participants instructions for completing their self survey.
  • E-mail participants instructions to forward to their selected raters for completion of a survey on the participant’s behalf.
  • Monitor status of surveys being completed on a weekly basis and report to client contact.
  • Act as a help center for any technical problems, providing assistance to client, participant and participant’s raters as needed.
  • Three (3) days prior to survey completion deadline, notify client of outstanding surveys that need to be completed.
  • One (1) day prior to survey completion deadline, notify client of outstanding surveys that need to be completed.
  • Prior to compiling reports, discuss with client the participants who did not receive a minimum of five (5) rater surveys. Determine whether to compile a report.
  • Once surveys are completed, compile results, download, print and assemble reports.
  • Client (or trainer or coach) will receive one report for each survey participant in a sealed, labeled envelope prior to the workshop or coaching session. Additional reports in electronic format may be purchased.
  • Client will be billed as follows [2006-2007 pricing]:
    • For each printed survey: 100% ($95 each participant)
    • Surveys not printed due to incomplete data: 50% ($50 each participant)
    • Canceled survey activity once launched: 50% ($50 for each participant
